29 CFR 1926.602(c)(1)(vii): Unauthorized personnel shall not be permitted to ride on powered industrial trucks. A safe place to ride shall be provided where riding of trucks is authorized. (a) Construction Site, West Side - On or about August 11, 2015 an employee was exposed to a fall hazard while being elevated by a Komatsu PC170LC Excavator (serial no. 30038). Employee was transported out of the trench in the bucket of this excavator.

29 CFR 1926.651(c)(2): Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. (a) Construction Site, West Side - On or about August 11, 2015 an employee was working inside a 6 feet 2 inches deep trench with no means of egress. An excavator was used to transport an employee out of trench.
