29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about December 27, 2019 at the worksite located at 4419 Rockrose Green Way (Lot#48) located in the Mundy Mill Subdivision in the city of Gainesville, Georgia employees were exposed to fall hazards of approximately 10.1 feet. Employees were installing roofing materials, the employer failed to ensure that each of the employees exposed to fall hazards of six (6) feet or greater were properly protected with fall protection measures according to the standards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about December 27, 2019, at the worksite located at 4419 Rockrose Green Way in Gainesville Georgia, employees accessing the upper landing area were exposed to fall hazards of approximately 10 feet, when the employer failed to ensure that the extension ladder used to gain access to the working area had the rails extending at least three feet above the landing area. No abatement certification or documentation is required for this item.
