29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:   On or about March 18, 2020 at the worksite located at 4349 Rockrose Green Way located within the Mundy mill Subdivision in the city of Gainesville Georgia employees using pneumatic nail guns to install roofing material were exposed to eye injuries when the employer failed to ensure that each employee was protected by proper eye and face protective systems.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March 18, 2020 at the worksite located at 4349 Rockrose Green Way located within the Mundy mill Subdivision in the city of Gainesville Georgia, employees installing roofing materials were subjected to injuries when the employer failed to ensure that the employees were properly protected from fall hazards with the use of proper fall arrest systems. Employees were utilizing "rock climbing" type body belts where the attachment point was in front of the pelvis area.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about March 18, 2020 at the worksite located at 4349 Rockrose Green Way located within the Mundy mill Subdivision in the city of Gainesville Georgia; employees were utilizing the material hoist tracks as a means of access and egress to the work area located on the roofing area of a new residential construction site.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.552(a)(4): 29 CFR 1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufacturers' recommendations.  On or about March 18, 2020 at the worksite located at 4349 Rockrose Green Way located within the Mundy mill Subdivision in the city of Gainesville Georgia; employees utilizing a ladder elevator to transport shingles to the roof were exposed to struck by hazards when the employer failed to ensure the material hoist ad the proper type rope installed where the prescribed wire type rope was replaced with a nylon "climbing" rope used to lift materials to the roof area exposing workers below to potential failure if the rope were to break or the knot loosen.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
