29 CFR 1910.178(l)(4)(ii)(A): Refresher training in relevant topics was not provided to operator(s) when the operator was observed operating the vehicle in an unsafe manner: (a) .Com Department/Warehouse located at 1803 Antioch Road, Dalton, GA 30720: The employer failed to retrain the operator after an incident occurred where an employee was elevated on the forks of a powered industrial truck and suffered a hand injury. Refresher training was not provided after the employer had knowledge that the operator had been using the equipment in an unsafe manner.
