29 CFR 1926.501(b)(6): Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems:    a) Employee(s) performing manual excavation & materials handling along the east side of FDNY Engine Co. 162 firehouse at 256 Nelson Ave. in Staten Island NY 10308.  Open excavation(s) were traversed by walking across a single wood plank (10" nom. width); exposed to hazard of falling 7 1/2 feet to the bottom of the excavation over either open-sided edge.    Condition(s) existed on or before 8/14/2015.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that the face of exposed soil wall(s) within the 7 1/2 ft deep excavation were vertical.      a) Employee(s) worked within excavation(s) of 7 1/2 ft depth below the sidewalk along the east outer wall of FDNY Engine Co. 162 firehouse at 256 Nelson Ave. in Staten Island NY 10308.  The soil walls were left vertical, and the timber shoring protective system used within the excavation was not designed in accordance with 1926.652 Appendix C, tabulated data from a shield system manufacturer, nor by a registered professional engineer; thus exposing employee(s) to hazard of soil cave-in.    Condition(s) existed on and before 8/14/2015.
