On or about August 19, 2015, on top of a residential roof, an employee using a Bostitch pneumatic nail gun was not wearing safety glasses, exposing himself to being struck in the face and eyes by nails or flying debris. R&K Certified Roofing of Florida, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 958909, citation number 1, item number 1, and was affirmed as a final order on March 11, 2014, with respect to a workplace located at 82 Florida Park Drive, Palm Coast, FL.

On or about August 19, 2015, employees were working on a 6:12 pitch roof completing reroof operations and were exposed to falls in excess of 6 feet or more because they were not protected by guardrail systems, safety nets, or personal fall protection systems. R & K Certified Roofing of Florida, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 958909, citation number 2, item number 1 and was affirmed as a final order on March 11, 2014, with respect to a workplace located at 82 Florida Park Drive, Palm Coast, FL 32137.
