On or about January 25, 2017, back side of home, a Werner 24-foot extension ladder had not been extended 3 feet past the landing, exposing employees to fall hazards.

On or about January 25, 2017, back side of home, employees were using a single rail of an extension ladder to gain access and exit from the roof. The ladder did not extend 3 feet and was not secured to prevent employees from falling 12 feet to the ground below.

R & K Certified Roofing of Florida, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1086539, citation number 01, item number 001 and was affirmed as a final order on October 1, 2015 with respect to a workplace located at 95 Cole Chester Lane, Palm Coast, FL 32137.
