29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:  a) On or about August 25, 2015 workers were exposed to cave-in hazards while performing work in a series of three trenches ranging from 6 to 8 feet in depth where hydraulic shoring (Efficiency brand) was not set up in accordance with tabulated data.  The northern-most trench only contained 1 shore.  The tabulated data requires a minimum of two shores for trenches less than 8 feet in length (this trench measured 6 ft in length).  Employees also worked at the ends of trenches where no end shores or other protective system was utilized to prevent cave-in of the vertical end walls of the trenches.   Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).  Elite Pipe MD, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1019869, citation number 1, item number 1a and was affirmed as a final order on March 19, 2015, with respect to a workplace located at 9560 Burgundy Dr., Littleton, CO 80123.
