29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement. On 8/27/2015, an employee and foreman from Allen Siding and Roofing were installing siding on the side of a residential building located on Wyndmere Lane in Lake St. Louis, MO. They were working on a pump jack scaffolding, that had poles erected on footings which were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement. All three of the poles supporting the pump jack scaffolding were not on level and sound footings to prevent displacement.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. On 8/27/2015, an employee and foreman from Allen Siding and Roofing were installing siding on the side of a residential building located on Wyndmere Lane in Lake St. Louis, MO. They were working on a pump jack scaffolding, which was not secured to the structure by rigid triangular bracing or equivalent at the bottom. Two of the three poles were on wood mudsills, which were sitting on sloped ground. Neither of the poles mudsills were staked to the ground to secure the poles. There was no physical connection, thus it was not an "equivalent means.
