Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On 8/27/2015, employees from Allen Roofing & Siding were installing shingles on the roof of a residential building located near the intersection of Wyndmere Drive & Schmitz Ct. in Lake St. Louis, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. The foreman and an employee were observed on the roof of the building without fall protection, exposing them to a fall to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On 8/27/2015, employees from Allen Roofing & Siding were installing shingles on the roof of a residential building located near the intersection of Wyndmere Drive & Schmitz Ct. in Lake St. Louis, MO. They were using a portable ladder improperly to access the roof. The ladder was not extended three feet beyond the landing or edge of the roof.
