29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: a) On or about August 25, 2015 - at a jobsite located at 26th & East End Avenue (Westside of the Street), Chicago Heights, IL. - an employee was performing sewer activities in a trench measuring eight (8) feet, three (3) inches deep, nine (9) feet long, ten (10) feet one (1) inch wide that was not provided with adequate cave in protection, thereby exposing the employee to cave in hazards.
