29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a. On or about September 30, 2019, at the above addressed jobsite, an employee was conducting sewer operations in a trench measured at seven (7) feet three in depth. The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards. Humir Construction, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1176159, citation number 1, item number 1 and was affirmed as a final order on November 3, 2016, with respect to a workplace located at Wilke Rd & Northwest Hwy., Arlington Heights, IL. Humir Construction, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1107816, citation number 1, item number 2 and was affirmed as a final order on January 5, 2016, with respect to a workplace located at 26th & East End Avenue, East side of the street, Chicago Heights, IL.
