29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c):       a)  On September 9, 2016, employees were not protected from the hazard of cave-in while working in an excavation measuring approximately 6.5 feet in depth.    Humir Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1107816, citation number 1, item number 2 and was affirmed as a final order on January 5, 2016, with respect to a workplace located at 26th & East End Avenue, East side of the street, Chicago Heights, IL 60411.    Humir Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1087847, citation number 1, item number 1and was affirmed as a final order on January 5, 2016, with respect to a workplace located at 26th & East End Avenue, West side of the street, Chicago Heights, IL 60411.    No abatement certification or documentation is required for this item.
