29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the jobsite located at 1540 SW Lakeside in Topeka, Kansas: Employees engaged in residential roofing activities from a 10:12 pitch roof were exposed to fall hazards of approximately 9.5 feet from the eave of the residence to the ground below in that the portable metal ladder used to access the roof was not secured to the residence and did not extend at least 3 feet above the upper landing.
