29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  On or about June 28, 2022, employees were exposed to a struck-by hazard of flying projectiles while using a pneumatic nail gun without the use of safety glasses.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  On or about June 28, 2022, and at times prior thereto,  employees were exposed to a fall hazard greater than 6 feet while installing the shingles on the roof of a residential construction site without being tied to an anchor for a personal fall protection system.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who might be exposed to fall hazards:  West side of the residential construction: On or about June 28, 2022, and at times prior thereto, employees were exposed to fall hazards greater than six feet when installing the roof shingles for a residential construction site without fall hazard recognition.
