29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On 12/5/2019, employees of Jose Luis Gonzalez were working on a residential building located at 2 Fairfax Drive, Saint Peters, Missouri.  Employees were using a pneumatic roofing nail guns without the use of eye protection.

29 CFR 1926.300(b)(2): Moving Part(s) of equipment exposed to contact by employees or which otherwise created a greater hazard, were not guarded.  On 12/5/2019, employees of Jose Luis Gonzalez were working on a residential building located at 2 Fairfax Drive, Saint Peters, Missouri.  Employees were exposed to caught in-between hazards while operating a Hitachi NV45AB2, 1.75 inch nail gun.  The guard covering the feeder assembly was removed, while in use.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 12/5/2019, employees of Jose Luis Gonzalez were working on a residential building located at 2 Fairfax Drive, Saint Peters, Missouri.  Employees were installing shingles approximately 9 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employees were observed on the roof of the building without fall protection.

The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.  On 12/5/2019, employees of Jose Luis Gonzalez were working on a residential building located at 2 Fairfax Drive, Saint Peters, Missouri.  The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.
