29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: a/ Deleted b/ Workplace floors around Wheelabrators in Buildings 1, 2 & 3: Employees were exposed to slipping hazards where there were excessive accumulations of steel blasting shots on workplace floors. c/ Kieserling area: Coiled up red hoses were left in the rear aisleway exposing employees to tripping hazards. d/ Building #2: Large steel rods supported by dunnage were stored along designated aisleway exposing employees to tripping hazards. e/ Twenty Ton Line: A flexible cord and a piece of 4" X 4" lumber were stored on a designated aisleway exposing employees to tripping hazards.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment: Building 1 and Draw Bench building: Work processes which exposed employees to airborne metal dusts while hand shoveling accumulated dusts in Wheelabrators and Dust Collection systems areas and dumping the dusts into larger containers were not evaluated to determine if personal protective equipment would be required.
