29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  a)  Repelletizer #2; On or about September 1, 2015 and at times prior; the employer exposed employees to potential hazardous energies in that employees were allowed to work on the Repelletizer #2 together without all employees applying a personal lockout device on the energy control lockout point.  a)  Thermoforming AL-1; On or about September 1, 2015 and at times prior; the employer exposed employees to potential hazardous energies in that employees were allowed to work on the Thermoforming AL-1 together without all employees applying a personal lockout device on the energy control lockout point.

29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:      a)  Extrusion department;  On or about September 1, 2015 and at times prior; the employer exposed his employees to a 12.10 feet fall hazard in that employees were allowed to walk in a platform with an unguarded portion in front of the access ladder when performing tasks such as verify the HVAC system.      Genpak, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(a)(2), which was contained in OSHA inspection number 315194811 citation number 1 item number 1 and was affirmed as a final order on June 2, 2011, with respect to a workplace located at 7621 Bill Joseph Parkway, Hope Hull, Al 36043

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section:  a)  Repelletizer #2;  On or about September 1, 2015 and at times prior, the employer exposed employees to hazardous energies (including electrical, pneumatic and hydraulic), caught-in and amputations hazards in that the machine specific lockout/tagout procedure did not adequately outline the procedural steps for placement, removal, transfer and verification of lockout or tagout devices to control all energy sources when employees perform service or maintenance on the  on the Repelletizer #2 at the extrusion department.  b)  Thermoforming AL-1;  On or about September 1, 2015 and at times prior, the employer exposed employees to hazardous energies (including electrical, pneumatic), caught-in and amputations hazards in that the machine specific lockout/tagout procedure did not adequately outline the procedural steps for placement, removal, transfer and verification of lockout or tagout devices to control all energy sources when employees perform service or maintenance on the Thermoforming AL-1#2 at the Thermoforming department.   Genpak, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 280226 citation number 1 item number 1 and was affirmed as a final order on August 1, 2012, with respect to a workplace located at 26 Republic Plaza Middletown, NY 10940.
