This exposed employees to fall hazards.  (b)  On or about September 9, 2015 at the 631 Hazel Street, Oshkosh, WI jobsite; an independent tie back safety line was not utilized while employees were performing window washing activities from The Rooftop Roller Suspended Solutions (no model or serial number) exposing employees to fall hazards.  (c)  On or about September 9, 2015 at the 631 Hazel Street, Oshkosh, WI jobsite; an independent tie back safety line was not utilized while employees were performing window washing activities from MIO Mechanical Corporation Portable Roof Rigger (Model:  MRR250A-2x8, S/N:  3524) exposing employees to fall hazards.
