29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person and the longest conductive object he or she could contact was guarded or at a distance of at least 10 ft. On a worksite located at 2295 S. Durkee Street, Appleton, WI 54911 an unqualified laborer employee was cleaning gutters and washing windows within 10 feet of overhead lines which were not deenergized or guarded from contact exposing the employee to potential shock or electrocution hazards.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On a worksite located at 2295 S. Durkee Street, Appleton, WI 54911, a laborer employee was exposed to falls of approximately 12.5 feet while cleaning gutters and washing windows on a flat roof of a residential house. The employee was not protected from falls with standard guard railing or a personal fall arrest system.
