OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees performing washing activities did not have the parapet clamp tied back to an anchor point: (a) On or about July 16, 2015 at the 700 N. Adams St., Green Bay, WI jobsite; an independent tie back safety line was not utilized when using parapet clamps while employees were performing window washing activities exposing employees to fall hazards.

Adams St., Green Bay, WI jobsite; while using parapet clamps lifelines were not utilized to an independent substantial member of the structure or rigged lines on the penthouse while performing dropping activities. This exposed employees to fall hazards.
