29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) CoorsTek, at 2449 Riverside Parkway, Grand Junction, CO: On and prior to August 30th, 2015, the employer had not ensured the point of operation between the laminating tape rollers on Mill #8 were guarded to prevent employees from having any part of the their body in the danger zone during operation. As a result, an employee's right index and middle finger tips were seriously injured after reaching into the roller assembly to retrieve a piece of broken laminating tape.
