29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:      a) On or about 09/10/15, in the driveway of a house at 39 Groveland St., Buffalo, NY: A ladder jack scaffold was erected to allow an employee to bring bundles of roofing shingles up to the roofers who were working in different areas of the roof. This employee walked back and forth on this scaffold and was exposed to a fall of approx. 17 foot. No fall protection system was utilized.      NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 09/10/15, on the roof of a house at 39 Groveland St., Buffalo, NY:  Four employees installing roofing shingles on a steep roof with a pitch of 8 in 12, were exposed to a fall of approx. 18 foot. All the employees were wearing full body harnesses, but none were tethered to an anchor point.      NO ABATEMENT CERTIFICATION REQUIRED
