29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 09/14/2015 - Tool crib, the polishing lathe was being operated with unguarded rotating parts. b) On or about 09/14/2015 - Tool crib, the Ikeda, radial drill press #7394 was being operated with unguarded rotating parts. c) On or about 09/14/2015 - Tool crib, the Harrison M390 lathe, model #M390GH, Serial #670390 was being operated with unguarded rotating parts. d) On or about 09/14/2015 - Tool crib, the Lagun FP2 vertical milling machine, serial #SE16933 was being operated with unguarded rotating parts.
