Combined Systems, Inc. 388 Kinsman Rd, Jamestown, Pa 16134- Building L- On or about September 11th, 2015 and times prior thereto: Employees periodically utilized Methylene Chloride for a cleaning process of the glue machine (Loctite RB10 Rotary Dispense Serial # 12CATA-13)and glue lines. The employer had not conducted personal air monitoring to determine employees exposure levels to Methylene Chloride.

Combined Systems, Inc. 388 Kinsman Rd, Jamestown, Pa 16134-Firing Range- On or about September 11th, 2015 and times prior thereto: Employees periodically performed work operations with and around Hexachloroethane during testing operations with #6210 HC smoke canisters. The employer did not evaluate the potential respiratory hazards, associated with smoke canister testing operations.
