29 CFR 1910.137(b)(2)(viii):  Electrical protective equipment was not subjected to periodic tests.  Test voltages and the maximum intervals between tests were not in accordance with Table I-5 and Table I-6.           (a)  Revere Plastics Systems, LLC:  On or about September 15, 2015, the employer did not ensure that the electrical protective gloves used by an employee when checking the heater band on the K cell were tested every 6 months.  The gloves were most recently tested on October 2013 and had been used by the employee since that date.  The employee was at risk of serious injury from electrical shock hazards while working with a 480 volts AC; 3-phase energized circuit.       (b)  Revere Plastics Systems, LLC:  On or about September 15, 2015, the employer did not ensure that the electrical protective gloves used by an employee when changing fuses on press #216 were tested every 6 months.  The gloves were most recently tested on July 2011 and had been used by the employee since that date.  The employee was at risk of serious injury from electrical shock hazards while working with a 480 volts AC; 3-phase energized circuit.
