29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Molding Machine 420 - 201: On or about March 30, 2021 and at times prior thereto, the employer exposed employees to struck-by and crush-by hazards in that the employer failed to ensure employees were utilizing the machine specific lockout tagout procedures prior to performing servicing/maintenance activities on the machine.
