On 9/15/2015, employees from A C Roofing Contractor LLC were observed tearing off shingles and installing sheathing on a roof of a residential building located on 20 Harbor Pointe Court in Lake St. Louis MO.  They were using a ladder improperly to access a second story landing.  The ladder was not extended to three feet beyond the landing surface.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems.    On 9/15/2015, employees from A C Roofing Contractor LLC were observed tearing off shingles and installing sheathing on a roof of a residential building located on 20 Harbor Pointe Court in Lake St. Louis MO.  They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.  The four employees were observed on the roof of the building without fall protection, exposing them to a fall to the ground below.    A C Roofing Contractor LLC was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926. 501(b)(1) which was contained in OSHA Inspection Number 98773, Citation 2, Item 2, issued on January 23, 2012, and became Final Order on February 17, 2012 with respect to a workplace located at 725 Lincoln Ave, Steamboat Springs, CO.
