29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, shall be 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level. When conditions warrant, the height of the top edge may exceed the 45-inch height, provided the guardrail system meets all other criteria of this paragraph. Note: When employees are using stilts, the top edge height of the top rail, or equivalent member, shall be increased an amount equal to the height of the stilts.  a.  On the second floor landing to Building 18, a top rail was not installed in front of the landing platform thereby exposing employees to a 13 foot fall hazard.  b.  On the second floor landing to Building 19, a top rail was not installed in front of the landing thereby exposing employees to a 13 foot fall hazard.

29 CFR 1926.503(a)(2): The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas:  29 CFR 1926.503(a)(2)(ii): The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used;  a.  On the site where employees were installing guardrail systems and working at heights which exceeded 6 feet, the employer did not provided fall protection training thereby exposing employees to a fall hazard.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, shall be equipped with:  29 CFR 1926.1052(c)(1)(ii): One stairrail system along each unprotected side or edge. Note: When the top edge of a stairrail system also serves as a handrail, paragraph (c)(7) of this section applies.  a.  On the stairway to Building 18, the employer did not install handrails to the left side of the stairway to prevent employees from falling through the openings beside the stairways thereby exposing employees to a fall hazard.  b.  On the stairway to Building 19, the employer did not install handrails to the left side of the stairway to prevent employees from falling through the openings beside the stairways thereby exposing employees to a fall hazard.
