29 CFR 1910.179(g)(3)(vii): Pushbuttons in pendant station(s) did not return to the off position when pressure was released by the crane operator: (a) Overhead crane in the rear section of the facility: on or about September 17, 2015: the employer failed to ensure that the controls for the crane used to move large concrete work pieces returned to the off position when released exposing employees to struck-by and crushed by hazards.
