29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Steel Shop/Reinforcing Department (Ironworker - Shear Area): On or about October 26, 2023, and at times prior, the employer exposed employees to amputation hazards in that point of operation front guarding was not provided to employees performing metal shear operations for production on the Scotchman Ironworker (Model No.: 12012, Serial No.: 5043M593). b) Steel Shop/Reinforcing Department (Ironworker - Punch Area): On or about October 26, 2023, and at times prior, the employer exposed employees to amputation hazards in that point of operation side guarding was not provided to employees performing metal punch operations for production on the Scotchman Ironworker (Model No.: 12012, Serial No.: 5043M593).
