29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a. In the recycling area, employees operating the Smart Products dismantler were exposed to the danger zone of the saw blade when pushing pallets through.

29 CFR 1910.213(r)(4): The mention of specific machines in paragraphs (a) thru (q) and this paragraph (r) of this section, inclusive, is not intended to exclude other woodworking machines from the requirement that suitable guards and exhaust hoods be provided to reduce to a minimum the hazard due to the point of operation of such machines.    a. In the cut shop, the green Whirlwind 212 chop saw used to cut wood for pallet manufacturing, was not adequately guarded at the point of operation. On or about September 14, 2015 the two hand control malfunctioned while an employee's hand was in the path of the saw blade causing an amputation to four fingers.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  Located at Pallet Distributors Inc. in Sugarcreek, OH:  a. On the 2015 OSHA 300 log, the employer did not include all the required information in column F:      1. Case number 3, CC on 8/28/15, did not include which fingers were injured, the actual injury and what caused the injury.     2. Case number 4, BC on 9/14/15, did not include what caused the amputation.
