29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: Cooper Farms, Inc. - St. Henry, OH: On September 16, 2015, the employer did not ensure that employees were protected from hazardous energy sources when cleaning the Automated Cutting Machine (ACM). Employees cleaning the machine were exposed to pinch points and rotating part hazards in that the electrical energy source which controls the mechanical movements was not shut down and isolated.
