29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operators and other employees from hazards created by ingoing nip points, rotating parts, flying chips and sparks: (a) Carestream Health, Inc. at 2000 Howard Smith Avenue West, Windsor, CO 80550: On September 3, 2015 guarding was not provided to protect employees that operate the 501 machine. Horizontal sheets of film rotate by means of powered rollers of speeds up to 345 feet per second. An employee attempted to remove a crease from a rotating roll of film by striking it with his hand. The employee was pulled onto the roll of film and traveled at least twice around it before another employee actuated the emergency stop button. The employee sustained a broken arm. This condition exposed employees to in-going nip point and caught in-between hazards. (b) Carestream Health, Inc. at 2000 Howard Smith Avenue West, Windsor, CO 80550: On September 3, 2015 guarding was not provided to protect employees that operate the 501 machine. Horizontal sheets of film rotate by means of powered rollers of speeds up to 345 feet per second. This condition exposed employees to in-going nip point and caught in-between hazards.
