29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  The employer did not provide training to ensure that the purpose and function of the energy control program were understood by authorized, affected, and other employees and that the knowledge and skills required for the safe application and usage of the energy controls were acquired by the following personnel:  a) Machine operators were not trained in hazardous energy control (lockout/tagout) procedures as required for authorized employees. Machine operators required to enter the machine area to lubricate dies and remove jams were exposed to unexpected start-up.This exposed employees to crushing, caught-in, and hazards capable of causing amputations.  b) On or about 18 September 2015, Coil Handlers were not adequately trained in hazardous energy control (lockout/tagout) procedures nor in the purpose and function of the program. An employee servicing scrap handling equipment was caught-in the machine during unexpected start-up which resulted in a partial arm amputation.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.217(f)(2): Instruction to operators. The employer shall train and instruct the operator in the safe method of work before starting work on any operation covered by this section. The employer shall insure by adequate supervision that correct operating procedures are being followed:  Mechanical power press operators were not properly trained, instructed and supervised on the safe method of work related to the function, use and limitations of the interlocked gate devices before work was performed. This exposed coil handlers and operators to crushing and caught-in hazards capable of causing amputation.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  Employees who performed servicing and maintenance tasks on and around mechanical power presses were not required to: (1) know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy,  (2) turn off and shut down the machines, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device, (5) prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished.  Employees required to rely upon electronic interlocks and related circuitry were exposed to caught-in and crushing hazards likely to cause amputation from unexpected start-up and energizing of part revolution mechanical power presses including, press #125.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.217(e)(1)(i)(A): Conduct periodic and regular inspections of each power press to ensure that all of its parts, auxiliary equipment, and safeguards, including the clutch/brake mechanism, antirepeat feature, and single-stroke mechanism, are in a safe operating condition and adjustment:    Periodic and regular inspections of each power press were not conducted to ensure that all of its parts and safeguards were in safe operating condition and adjustment. The employer did not ensure that the presses did not automatically start up when the interlocked doors were closed. This exposed employees to caught-in and crushing hazards.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
