On or about September 5, 2019, two employees were performing adjustments in the die area of the press and did not each affix a personal lockout device to the group lockout device before performing work inside the open die area.  A third employee had affixed his lock to a hasp on the disconnect switch, but had left the work area.  Employees were exposed to potential crushing and amputation hazards to their hands and arms in the event of the unexpected startup of the press.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

On or about September 5, 2019, employees who retrieved and stacked the finished foil pans at the discharge end of the line were exposed to an ingoing nip point between the belt and pulley at the end of the conveyor where the yellow guard was not seated properly.    Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).
