29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about September 16, 2015, the employees were exposed to injuries from falls approximately 13.0 feet while engaged in roofing activities.  Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").  Rendon Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 954602, citation number 1, item number 1 and was affirmed as a final order on April 25, 2014, with respect to a workplace located at 1302 Lookout Springs, Colorado Springs, Co 80921.  Rendon Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 592423, citation number 1, item number 1 and was affirmed as a final order on September 7, 2012, with respect to a workplace located at 310 5th Ave, Lyons, Co 80540.  Rendon Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) (name previously cited standard), which was contained in OSHA inspection number 106654, citation number 1, item number 2 and was affirmed as a final order on October 31, 2011, with respect to a workplace located at 1620 S. Wadsworth Blvd., Lakewood, Co 80232.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a)  On or about September 30, 2015, an employee was exposed to struck-by hazards due to not wearing eye protection while utilizing a pneumatic nail gun to secure roofing materials on the roof.
