29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports. (a) Chimney - On or about September 30, 2015 scaffold planks were moved aside in order to have an opening in the platform to pass mortar from the third tier to the fourth tier platform. The work platform was not fully planked or decked. An employee was observed straddling the platform opening.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along open sides and ends of platforms: (a) Chimney - On or about September 30, 2015 a scaffold used for bricking a chimney did not have standard guardrails at the ends of bracket platform. The brackets were attached to the fifth tier of the scaffold. Employees were exposed to falling 30 feet to ground level.

29 CFR 1926.451(e)(6): Integral prefabricated scaffold access frames did not meet the requirements of paragraphs (e)(6)(i)-(vi) of this section: (a) Chimney - On or about September 30, 2015 employees were accessing a scaffold platform by climbing on the end frame that was not designed to be an access ladder. The River City Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(6), which was contained in OSHA inspection number 561878, citation number 1, item number 2 and was affirmed as a final order on September 17, 2012, with respect to a workplace located at 1910 Main St., Madison MS 39110.
