29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about 10/05/15, on the steep roof of a house at 33 Mill St., Williamsville, NY: An employee was exposed to a potential fall of approx. 30 ft. to the ground while on a steep roof doing masonry work on a chimney. The employee was wearing a full body harness, but was not tethered to anything. No other fall protection system was utilized to protect the employee. b) On or about 10/05/15, on the flat roof section at the southeast part of the house at 33 Mill St., Williamsville, NY: An employee was exposed to a potential fall of 10 ft. 10 in. to the ground while on the flat roof , then climbed a ladder to supply a container of concrete to the employee working on the chimney. No fall protection system was utilized to protect the employee. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a) On or about 10/05/15, at the house at 33 Mill St., Williamsville, NY: An employee accessed the flat roof section at the southeast part of the house by climbing a scaffold section, walking north towards the house on a single scaffold pik, then hoisting himself up backwards a distance of 3 ft. to the lower roof, exposing himself to a potential fall of approx. 8 ft. to the ground . No ladder or other proper means of access to the lower roof was provided. NO ABATEMENT CERTIFICATION REQUIRED
