29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) On or about 6/11/19, on a chimney repair jobsite at a house located at 699 Lafayette Avenue, Buffalo, NY:  An employee engaged in chimney repair was exposed to a fall of approx. 30 ft. while standing on a wooden plank supported by brackets that extended on the outside of the tubular welded frame scaffold that was set up on the left side of the house from the ground to the top of the chimney. No guardrail system, or personal fall arrest system was utilized.     NO ABATEMENT CERTIFICATION REQUIRED
