C & D Waterproofing Corporation, 301 West Main St., Clarksburg, WV: On or about 10/05/15, the vertical lifelines that employees were using as part of a personal fall arrest system were tied to the same horizontal lifeline used by the suspension scaffold. ABATEMENT CERTIFICATION IS REQUIRED.
