29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a)  On or about October 6, 2015, the employer did not conduct annual audits of its energy control procedure to assure that proper lockout/tagout procedures and requirements were being followed.  The lack of procedure audits exposed employees to amputation injuries in that adequate machine isolation was not being followed when employees accessed hazard areas of machines while performing tool and die changes.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):     a)  On or about October 6, 2015, the employer did not ensure that employees isolated the electrical, gravitational and pneumatic energy sources when performing die changes on the Minster 7 mechanical power presses.  Employees were exposed to crushing hazards while performing this operation.     b)  On or about October 6, 2015, the employer did not ensure that employees isolated the electrical and pneumatic energy sources when performing tooling changes on the screw and chucker machines.  Employees were exposed to caught in hazards from the rotating parts while performing this operation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a)  On or about October 6, 2015, the employer did not ensure that the screw machines were adequately guarded to prevent employees from accessing the rotating parts during operation and while loading bar stock.  The screw machines were equipped with moveable barrier guards which had large openings due to broken Plexiglas or broken plastic sheeting and were not equipped with safety interlocks or other means to stop the machine when the door were opened.  Employees were exposed to caught-in hazards from the moving equipment.

The employer shall establish and follow an inspection program having a general component and a directed component.    a)  On or about October 6, 2015, the employer failed to establish and follow a program of periodic and regular inspections for the Minster 7 mechanical power presses to ensure that all parts, auxiliary equipment, and safeguards of the press were in a safe operating condition and adjustment. Employees were exposed to amputation injuries due to the lack of press inspections.

The employer did not insure by adequate supervision that correct operating procedures are being followed:     a)  On or about October 6, 2015, the employer did not ensure that employees were trained in the safe operation of Minster 7 mechanical power presses (75 Tons) located in the press room. Training was not provided regarding the hazards associated with key used to control the mode selection. Operators were exposed to amputation injuries.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:    a)      On or about October 6, 2015, the employer did not assure that employees were protected from caught in hazards associated with the chain and sprocket on the front of the Minster 7 mechanical power press (#4043).  The operator stands in the area to watch the press which was several inches away from the chain and sprocket.
