29 CFR  1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8-hour Time-Weighted Average (TWA) sound level of 85 dBA:   Galion, LLC located at 515 N. East Street in Galion, Ohio: On or about June 13, 2023, employees working within the ACME room were exposed to noise greater than 90 decibels. Failure to implement an effective hearing conservation program for employees whose exposures are greater than 85 decibels, exposes employees to Standard Threshold Shifts, chronic hearing loss, and other irreversible audiometric conditions.

29 CFR  1910.95(k)(1):The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k):  Galion, LLC located at 515 N. East Street in Galion, Ohio: On or about June 13, 2023, employees working within the AMCE room were exposed to noise greater than 85 decibels measured on a 8-hour Time Weighted Average. Failure to ensure employees trained on the hazards associated with hazardous noise and participate with the hearing conservation program can contribute to chronic hearing loss, Standard Threshold Shifts, and other irreversible noise-related illnesses.

29 CFR  1910.178(f)(2):The storage and handling of liquefied petroleum gas fuel was not in accordance with NFPA Storage and Handling of Liquefied Petroleum Gases (NFPA No. 58-1969), which is incorporated by reference as specified in Sec. 1910.6:  Galion, LLC located at 515 N. East Street in Galion, Ohio:  On or about April 6, 2023, employees working within the maintenance bay were exposed liquified petroleum gas tanks that were not properly stored. The liquified petroleum gas tanks were removed from powered industrial vehicles which were being serviced by the maintenance technician. Failure to safely store liquefied petroleum gas tanks exposes employees to potential fire and/or explosion hazards.

East Street in Galion, Ohio: On or about February 14, 2023, the employer did not ensure that each operator was competent to operate powered industrial vehicles demonstrated through successful completion of training and evaluation requirements. Failure to ensure employees are competent to operate powered industrial vehicles exposed an employee to struck-by hazards while operating the PIV near the CNC room.

29 CFR  1910.178(q)(1):Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  Galion, LLC located at 515 N. East Street in Galion, Ohio: On or about April 6, 2023, the employer did not ensure that powered industrial vehicle that is not in a safe operating condition was removed from service. Powered industrial vehicle Yale #G7 (S/n E187V21263A) was observed to have damaged hydraulic lines. Failure to remove PIVs that are in a disrepair condition exposes employees to crushed-by or struck-by injuries in the event of a hydraulic system failure.

29 CFR  1910.253(e)(6)(ii): Where regulators or parts of regulators, including gages, need repair, the repair should be made by a qualified person.  Galion, LLC located at 515 N. East Street in Galion, Ohio: On or about April 6, 2023, the employer did not ensure that regulators on the oxygen-acetylene torches in need of repair were removed from service and repaired by a qualified person. The oxygen-acetylene torches located in the maintenance bay were discovered to have defective regulators, in that the gauge lens was broken. Failure to ensure regulators in need of repair were repaired by a qualified person exposes employees to potential fire and explosion hazards during use of the equipment.

29 CFR  1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:   Galion, LLC located at 515 N. East Street in Galion, Ohio: On or about April 6, 2023, the employer did not ensure that the damaged electrical box used to power the ESAB EMP 215ic was free from recognized hazards likely to cause death or serious harm to employees. Employees within the maintenance bay utilized a metal junction box affixed to a flexible cord as part of the power supply for the ESAB EMP 215ic welder. Failure to remove damaged electrical equipment from service exposed employees to electrical shock and burn hazards.
