Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

Employees operating in this condition were exposed up to a 16 ft. fall to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a)  Arturo Lopez on or about October 08, 2015, at the address of 1927 Mason St. Toledo, OH: The employer failed to ensure the side rails of both extension ladders used to access the work area were extended at least three feet above the landing/working level. Employees utilizing the ladder in this condition were exposed to falls of approximately 16 ft.

Employees were exposed to potential impact injuries caused from falling material and/or equipment.
