29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Arturo Lopez DBA Arturo Roofing on or about April 25, 2017, at the address of 8205 Jerusalem Rd. Curtice, OH: The employer did not ensure employees were protected from falls while installing roofing material on a residential type structure. Employees operating under this condition were exposed up to a 19 feet fall to the ground below. Arturo Lopez DBA Arturo Roofing was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1097089, citation number 1, item number 1b and was affirmed as a final order on January 25, 2016, with respect to a workplace located at 1927 Mason St. Toledo, OH.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Arturo Lopez DBA Arturo Roofing on or about April 25, 2017, at the address of 8205 Jerusalem Rd. Curtice, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, personal fall arrest systems or alternative measures while performing residential roofing operations. Arturo Lopez DBA Arturo Roofing was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1097089, citation number 1, item number 1c and was affirmed as a final order on January 25, 2016, with respect to a workplace located at 1927 Mason St. Toledo, OH.
