29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors. a. The employees were using flexible cord equipment that was not protected by either a GFCI or assured grounding program, thereby exposing employees to an electrical hazard.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. a. The flexible cord for the circular saw was damaged below the strain relief, thereby exposing employees to an electrical hazard. Alderete Construction was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 945446, citation number 1, item number 3 and was affirmed as a final order on 4 December 2013, with respect to a workplace located at 1231 Gabrielle Elaine Drive, Columbus, OH 43228.

Each employee on a steep roof with unprotected sides and edges six feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. Employees were observed framing a roof with a pitch of 5 in 12 without being tied off, thereby exposing employees to a fall hazard of up to 40 feet. Alderete Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 945446, citation number 1, item number 4 and was affirmed as a final order on December 4,2013, with respect to a workplace located at 1231 Gabrielle Elaine Drive, Columbus, OH 43228.
