29 CFR 1926.652(g)(2): Excavations of earth material to a level greater than 2 feet (.61 m) below the bottom of a shield was permitted:     A. Ohio Excavating & Sewer Contractors Corp. worksite located on Roachton Rd. (The Village at Riverbend), Perrysburg, Ohio: On or about October 7, 2015, the employer did not ensure the bottom of the protective trench shield was within 2ft from the base of the excavation. An employee was exposed to potential wall failure when the bottom of the protective shield was 3ft above the bottom of the excavation while installing the 30 inch drainage pipe.
