29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    (a) On the second floor of building number 5, west: On or about October 7, 2015, and at times prior thereto, employees were exposed to a fall hazard greater than 6 feet while building and erecting exterior walls on the second floor of a multifamily complex under construction without being trained on fall hazard recognition.   (b) On the second floor of building number 4, west: On or about October 7, 2015, and at times prior thereto, an employee was exposed to a fall hazard greater than 6 feet while snapping chalk lines on the second floor of a multifamily complex under construction without being trained on fall hazard recognition.   (c) On the second floor of building number 3, south: On or about October 7, 2015, and at times prior thereto, employees were exposed to a fall hazard greater than 6 feet while installing floor trusses on the second floor of a multifamily complex under construction without being trained on fall hazard recognition.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:       On the second floor of building number 3, south: On or about October 7, 2015, and at times prior thereto, employees were exposed to an electrical hazard while using a damaged electrical extension cord plugged into a circular saw that was used to cut blocks.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:      (a) On the second floor of building number 5, west: On or about October 7, 2015, and at times prior thereto, employees were exposed to a fall hazard greater than 6 feet while building and erecting exterior walls on the second floor of a multifamily complex under construction without using fall protection.
