29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    East side of building next to Walters St:  On or about April 29, 2016 an employee was exposed to a struck-by hazard while using a Max Decking Coil Nailer, Model CN665, without eye protection, while attaching exterior sheathing to the building.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.  East side of building next to Walters St:  On or about April 29, 2016 an employee was exposed to a fall hazard greater than 6 feet to the dirt ground below, while using a JLG aerial lift, Model 600S, without fall restraint protection, while accessing the exterior of the building while installing sheathing.
