29 CFR 1926.451(a)(6): Scaffolds shall be designed by a qualified person and shall be constructed and loaded in accordance with that design. On 10/01/2015, an employee and foreman from Allen Siding and Roofing were installing siding on the side of a residential building located on lot 32 located at the intersection on Scenic Drive and Cedar Mountain Court in St. Peters, MO. The employee was installing siding from a pump jack scaffolding which was attached to a ladder jack scaffolding. The plank for the ladder jack scaffolding was supported by a ladder and bracket on one end and the other end of the plank was laying on top of the plank for the pump jack scaffolding. The scaffolding was not designed or constructed by a qualified person and loaded in accordance with that design.

On 10/01/2015, an employee and foreman from Allen Siding and Roofing were installing siding on the side of a residential building located on lot 32 located at the intersection on Scenic Drive and Cedar Mountain Court in St. Peters, MO. The employee was working on a pump jack scaffolding that had poles erected on footings which were not level and capable of supporting the loaded scaffold without settling or displacement. Two out of three poles supporting the pump jack scaffolding were not on level and sound footings to prevent displacement.
